摘要
目的 比较立体定向血肿引流术（定向引流术）和锁孔血肿清除术（锁孔术）治疗高血压脑出血的效果和适用范围。方法 观察44例定向引流术和70例锁孔术各项临床指标、近期（术后1个月）和远期（术后6个月）随访结果。结果 术后并发症发生率、近期预后优良率、近期死亡率、远期预后优良率、远期死亡率在定向引流术组和锁孔术组中分别为：29．5％和35．7％，36．4％和45．7％，4．6％和12．9％，64．3％和65．6％，4．8％和1．6％，差别无统计学意义。结论 两种术式的疗效没有明显差别。
Objective To compare stereotactic puncture SP group and key－hole craniotomy evacuation KE group in the treatment of hypertensive cerebral hematomas． Method A prospective trial was conducted for comparing the impact of two surgical techniques mentioned above on surgical results and patients＇ outcome． Result SP was used in 44 patients and KE in 70 patients,the post－operative complication rate was 29．5％ and 35．7％,the good prognosis rate was 36．4％ and 45．7％ 1 month after surgery,64．3％ and 65．6％ 6 months after surgery respectively in the SP group and the KE group． There was no significant difference between both groups． Conclusion The stereotactic puneture and key－hole craniotomy are equal to be effective in the treatment of hypertensive cerebral hematomas． There was no difference in the complications and long－term outcome between the two methods．
