严重烧伤患者应用重组人生长激素对糖和蛋白质代谢的影响 被引量:25

The influence of recombinant human growth hormone on the metabolism of glucose and protein in severely burned patients

摘要 目的 分析重组人生长激素(rhGH)对严重烧伤患者血糖及蛋白质代谢的影响。方法对24例应用rhGH治疗的严重烧伤患者的血清总蛋白、白蛋白及血糖升高发生率进行比较分析。并对同期未用rhGH患者血糖及蛋白质的变化进行对比分析。结果 严重烧伤患者应用rhGH后血清总蛋白、白蛋白较用药前明显升高,并高于同期对照组。伤后2周内血糖升高,特别是伤后1周内明显升高,伤后0-3 d、4-7 d、8-14 d的血糖升高发生率分别为52.38%、50.24%、20.00%。伤后 1周内、2周内及2周后开始应用rhGH其血糖升高发生率分别为100%、50%、0。结论 严重烧伤患者应用rhGH能促进机体蛋白质合成,但伤后早期应用rhGH对血糖影响较大,伤后2周应用rhGH是较合适的时机。 Objective To investigate the influence of recombinant human growth hormone(rhGH ) on the metabolism of glucose and protein in severely hurned patients. Methods The serum levels of total pro- tein and albumin and the incidence of hyperglycemia in 24 cases of severely burned patients treated by rhGH were determined and compared with those in patients without rhGH treatment (control group). Results The serum levels of total protein(TP) and albumin in burned patients after being treated by rhGH were much higher than those before treatment and those in control group. The blood level of glucose increased within 2 weeks , especially 1 week postburn. The incidences of hyperglycemia were 52. 38%, 50. 24% and 20% on 0 ~ 3,4 ~ 7 and 8 ~ 14 postburn days (PBDs ) , respectively. And the incidences of hyperg1ycemia were 100%, 50% and 0% when the administration of rhGH started within 1,2 and after 2 weeks respectively. Conclusion The systemic protein synthesis in severely burned patients could be accelerated by the use of rhGH. But early administration of rhGH in burned patients could affect blood sugar level obviously. And the optimal time to give rhGH is 2 weeks after burn injury.
