Effects of growth hormone on the adaptation of remnant bowel after massive small bowel resection

摘要 目的:研究生长激素对大鼠大部小肠切除后残存小肠的代偿作用,并探讨其作用机制. 方法:雄性SD大鼠30只,随机分成三组,即对照组(行小肠切断吻合术)、短肠组(行80%小肠切除术)和生长激素组(行80%小肠切除术+术后皮下注射生长激素1 U/kg@d-1×28天).术后第28天活体取残存小肠粘膜分别进行病理切片图像分析、流式细胞检查、增生细胞核抗原测定. 结果:生长激素组的增生细胞核抗原、肠粘膜S期细胞比率系数和增生指数较短肠组显著增高(P<0.05),而短肠组与对照组相比虽有所升高,但并无显著差异(P>0.05). 结论:对切除80%小肠的SD大鼠术后应用外源性生长激素1 U/(kg@d),S期细胞比率系数、增生指数和增生细胞核抗原表达增强,表明应用外源性生长激素可明显促进术后残存小肠粘膜的增生. Objectives:To evaluate the effects of growth ho rmone on remnant bowel after massive small bowel resection. Methods:Thirty male Sprague-Dawley rats were randomly divided into three experimenta l groups: the control group(small bowel transection and anastomosis),the short bo wel group(80% small bowel resection) and the growth hormone group(80% small bowe l resection and hypodermic injection of growth hormone 1U/kg@d-1×28 d a fter the operation).On the 28th day after the operation, remnant small bowel muc osa were harvested for pathological, flow cytometry, and PCNA analysis. Results:The villus height(VH) of small bowel mucosa was significantly hi gher in the growth hormone group than that in the short bowel group, and the lat ter was significantly higher than that in the control group; Crypt depth and muc osa thickness were not significantly higher in the growth hormone group than tha t in the short bowel group, but they were significantly higher than those in the control group; PCNA, synthesis part function and proliferation index were signi ficantly higher in the growth hormone group than those in the short bowel group( P <0.05); PCNA expression in the short bowel group was significantly higher t han that in the control group( P <0.05).The synthesis part function and prolif eration index in the short bowel group were higher than that in the control grou p, but the differences were not significant( P >0.05). Conclusion s:Analysis of the villus height, crypt depth, mucosa thickness, synthesis part function, proliferation index and PCNA revealed that growth hormone could accel erate the proliferation of mucosa cells in the remnant small bowel.
