22例眼球钝挫伤致葡萄膜炎临床分析

Clinical Analysis on 22 Cases of Uveitis Caused by Abortive Injury in Eye-ball

目的：探讨眼球钝挫伤致葡萄膜炎的发病机理及临床疗效。方法：对 22例 24眼眼球钝挫伤所致葡萄膜炎患者进行分析 (平均年龄 22. 5岁，男性 21例占 95. 45％，女性 1例占 4. 55％ )，单眼 20例占 83. 3％，双眼 2例占 16. 6％。视力 0. 8～ 0. 5者 10眼， 0. 3～ 0. 1者 11眼，小于 0. 1者 3眼。全部患者诊断明确后用皮质类固醇、消炎痛及散瞳等治疗。结果： 22例 24眼 (100％ )症状消失， 13眼 (54. 17％ )体征消失，裸眼视力大于等于 1. 0者 4眼 (16. 67％ )， 0. 5～ 0. 9者 14眼 (58. 33％ )， 0. 1～ 0. 3者 7眼 (29. 17％ )，小于 0. 1者 1眼 (4. 17％ )。结论：眼球钝挫伤致葡萄膜炎主要是由于伤后前列腺素增加和组织胺、 5－羟色胺、缓激肽等生物活性介质及一些炎性因子被启动、激活、释放所致。伤后须注意早期眼部体征，及时诊断、合理使用皮质类固醇、消炎痛及散瞳治疗。

Objective:To discuss the pathogenic mechanism and the therapy effect of the uveitis caused by abortiove injury in eye-ball.Methods:To analyses 24 eyes on 22 cases of uveitis caused by abortive injury in eye-ball .Of the 24 eyes,the vision of 0.8～ 0.5 were account for 10 eyes,0.3～ 0.1 occupied 11 eyes and 0.1 was less than 3 eyes.All the cases were received cocorticoids,indomethacin and mydriasis after diagnosed,Results:The symptom of 24 eyes in 22 cases were disappeared(100％ )and the signs of 13 eyes were also disappeared(54.17).The naked-vision of 4 eyes were more than 1.0,and 14 eyes were 0.5～ 0.9,7 eyes were 0.1～ 0.3,only l eye was less than 0.1.Conclusion:It should pay more attention to the early signs of the eyes and make diagnosis in time,as well as use cocorticoids,indomethacin and mydriasis suitably in treating uveitis caused by abortiove injury in eye-ball.
