复杂的外伤性白内障手术361例临床观察

Clinical observation on 361 cases of complexed traumatic cataract surgery.

目的 探讨伴有眼球其他组织外伤的复杂的外伤性白内障的手术方法。方法 各类复杂的外伤性白内障 361例 364眼 ,根据外伤性白内障及其他组织外伤的不同情况 ,分别采取伤口修复、眼前段结构重建、前路晶状体玻璃体切除、眼内异物摘出联合白内障摘出及人工晶状体植入术 ,术后观察并发症及出院时视力。结果 术后视力 <0 0 5者 72眼 ,0 0 5～ 0 2者 18眼 ,0 3～ 0 4者 10 7眼 ,0 5～ 0 9者 12 8眼 ,≥ 1 0者 2 6眼 ;儿童视力检查不合作者 13眼。结论 对伴有其他组织外伤的复杂的外伤性白内障采取相应的联合手术方式 ,可获得满意的效果。

Objective To study the methods of surgery for traumatic cataract and accompanied with other tissue injuries of the eye.Methods According to the different conditions of traumatic cataract and accompanied other tissue injuries,the different measures including wound repairation,reestablishment of the anterior segment of the eye,lensectomy and vitrectomy through limbal incision,intraocular foreign body extraction with cataract extraction and intraocular lens implantation were taken.Results The postoperative visual acuity is less than 0.05 in 72 eyes,0.05～ 0.2 in 18 eyes,0.3～0.4 in 107 eyes,0.5～0.9 in 128 eyes and ≥1.0 in 26 eyes.Visual acuity examination was not accomplished in 13 eyes of children.Conclusion With a combined surgery,satisfactory therapeutic result can be obtained for comlexed traumatic cataract and accompanied with other tissue injuries.
