角膜层间植入羊膜治疗大泡性角膜病变

Amniotic membrane transplantation in intercornea stroma for bullus keratopathy.

目的 探讨角膜层间植入羊膜在治疗大泡性角膜病变 (bullouskeratopathy ,BK)中的应用价值。方法 对 2 1BK患者进行角膜层间植入羊膜手术治疗 ,术中先用 8 8～ 9.0mm的环钻在角膜上环切 1/2角膜厚度 ,做一以 5～ 6～ 7点为蒂的 1/2厚的角膜瓣 ,将制好的与角膜瓣相同大小的羊膜置于角膜层间铺平 ,用 10 0尼龙线间断缝合角膜瓣。结果 2 1例患者疼痛等刺激症状消失 ,术后 3～ 6天羊膜呈灰白色水肿 ,随后逐渐透明。经 3～ 12个月的随访观察 ,均未发现BK复发及并发症的发生。结论 角膜层间植入羊膜可有效地控制BK的症状 ,防止BK复发。

Objective To investigate the value of amniotic membrane transplantation in intercornea stroma for bullous keratopathy.Methods Twenty one patients with bullous keratopathy were treated with amniotic membrane transplantation in intercornea stroma.One second thick cornea flap with bases on 5～6～7 o′clock was made with 8.8～9.0 mm corneal trephin.Then an amniotic membrane same size as the cornea flap was spreaded in intercornea stroma and the cornea flap was sutured with 10 0 Nylon suture interruptedly.Results The symptoms of pain and irritation disappeared after the treatment.3～6 days postoperation amniotic membranes looked like edema with greyish white colour,then became clear gradually.Bullous keratopathy recurrence and complications were not found during 3～12 months follow up.Conclusion Amniotic membrane transplantation in intercornea stroma is effective to control the symptom of bullous keratopathy and prevent bullous keratopathy recurrence.And it is one effective method of treatments for bullous keratopathy.
