摘要
目的评价呼气末正压通气(PEEP)治疗老年急性心源性肺水肿的临床疗效。方法15例老年急性心源性肺水肿患者,经常规抗心衰药物治疗无效后,加用PEEP(3～8cmH2O)机械通气治疗,对比治疗前后血压(BP)、呼吸(R)、心率(HR)、氧饱和度(SaO2)等动脉血气指标、胸片及心功能变化。结果经PEEP机械通气治疗30min～1h后,14例患者呼吸频率、心率减慢,氧饱和度上升,动脉血气指标好转,心功能改善。仅1例无效死亡。总有效率93.33%。结论呼气末正压通气是治疗老年急性心源性肺水肿快速、有效的方法。
Objective To evaluate the clinical effect of me chanical ventilation with PEEP in th e treatment of acute cardiogenic pulmonary edema in the elderly.Methods 15aged patients with acute cardioge nic pulmonary edema were treated by mechanical ventilation with 3～8cmH 2 O PEEP when the conventional drugs had no effect.The BP,R,HR,SaO 2 ,blood gas analysis,chest X -ray and cardiac function were compared before and after PEEP treatment.Results After PEEP treatment 14patients were recovered with no more dyspnea ,tachycardia an d hypoxemia.One patient died.The efficacy was 93.33%.Conclusions Mechanical ven-tilation with PEEP is feasible and effective in the treatment of acute cardiogenic pulmonary edema in the elderly.[
