腓肠神经营养血管岛状皮瓣的临床应用

Clinical Application of Island Flaps Incorporating the Nutrient Vessel of Sural Nerve

目的 总结腓肠神经营养血管岛状皮瓣的临床应用效果。方法 1998年开始应用腓肠神经营养血管岛状皮瓣修复膝、小腿、踝及足部皮肤软组织缺损 18例 ,其中顺行蒂 3例 ,逆行蒂 15例。皮瓣面积 5cm× 7cm～ 8cm× 16cm。血管蒂长 9～ 16cm ,蒂宽 2～ 3cm。结果 术后 17例皮瓣完全成活 ,1例皮瓣尖端部分皮肤坏死 ,经换药愈合。经 3个月～ 1年随访 ,皮肤质地及外形较好。结论 该皮瓣血供可靠 ,不牺牲肢体主干动脉 ,操作简单 ,是修复膝、小腿、踝及足部皮肤软组织缺损较好方法。

Objective To sum up the application experience of the island flaps incorporating the nutrient vessels of sural nerve. Methods From 1998, the island flaps incorporating the nutrient vessels of sural nerve were performed to repair the soft tissues defects of the knee, shank, ankle and foot in 18 cases, 15 cases with a distal base and 3 cases with a proximal base. The range of the flaps were 5?cm×7?cm to 8?cm×16?cm. Results Seventeen flaps survived completely, one flap was partially necrosis and the wound healed after changing dressing. Followed up 3 to 12 months, the flaps had fine texture and appearance. Conclusions The flaps have advantage as follow: the reliable blood supply, avoidance of sacrificing the major artery and simple procedure. It is a good method to repair the soft tissues defects of the knee, shank, ankle and foot.
