雷公藤多甙对哮喘患者的抗炎和平喘作用的机理研究

Study on Anti-inflammatory and Antiasthmatic Action Mechanism of Glucosidorum Triplergii Tororum(GTT) in Asthmatic Patients

目的 :探讨雷公藤多甙 (GTT)对哮喘患者的抗炎、平喘作用机理。方法 :选择 60例过敏性哮喘患者随机分为GTT治疗组和对照组 ,治疗前后分别采用ELASA法检测PBMC培养上清液中白介素 4 (IL 4 )、γ 干扰素 (IFN γ)、可溶性白介素 2受体 (sIL 2R)、可溶性IgE低亲和力受体 (sCD2 3)含量 ,采用逆转录PCR检测IL 4 mRNA表达量 ,采用嗜碱性细胞脱颗粒试验检测嗜碱性细胞释放能力 (BRA)。结果 :哮喘患者上清液中IL 4、sIL 2R、sCD2 3含量、IL 4 mRNA表达量及BRA均显著增高 ,而IFN γ含量显著减少。治疗组经GTT治疗后 ,上清液IL 4、sIL 2R、sCD2 3含量、IL 4 mRNA表达量及BRA均显著降低 ,而IFN γ含量显著增高。对照组治疗前后上述指标均无显著差异。结论 :哮喘患者T细胞和B细胞呈激活状态 ,TH1和TH2细胞因子之间平衡失调 ,以TH2细胞因子占优势。GTT能显著抑制哮喘患者T细胞和B细胞的活化 ,能纠正体内TH1和TH2细胞因子网络的失平衡状态 ,并能降低BRA。GTT可能通过上述机理抑制气道变应性炎症 ,从而达到平喘的效果。

Objective To explore the mechanism of anti inflammatory and anti asthmatic action of GTT in asthmatic patients.Methods Sixty patients with allergic asthma were randomly divided into GTT treated group and control group.IL 4,IFN γ,sIL 2R,sCD23 levels in culture supernatant of PMBC were determined by ELISA,expression of IL 4mRNA by RT PCR,Basophil releasability(BRA)by basophil degranulation test.Results Supernatant IL 4,sIL 2R,sCD23 levels,expression of IL 4mRNA and BRA in asthmatics were significantly increased,IFN γ level was significantly decreased.After treatment of GTT,supernatant IL 4,IL 2R,sCD23 levels,expression of IL4 mRNA and BRA were significantly decreased,IFN γ level was significantly increased.Above parameters in control group had not significant difference between before and after treatment.Conclusion T and B cell of asthmatics are activated.Th1 and Th2 cytokine network is in imbalance state,Th2 cytokine has predominated.GTT may suppress T and B cell activation and correct the imbalance state of the Th1 and Th2 cytokine network,by which GTT may inhibit allergic airway inflammation,thereby control asthma.
