摘要
目的 :探讨在颈椎间盘纤维环及髓核退变中组织形态的变化。方法 :对正常人、单纯颈椎间盘突出症、脊髓型颈椎病三组椎间盘纤维环及髓核进行电镜观察。结果 :三组椎间盘胶原纤维无明显变化 ,单纯颈椎间盘突出症与脊髓型颈椎病人的退变椎间盘细胞较正常人有明显变化 ,表现为严重退变或细胞坏死。结论 :单纯颈椎间盘突出症患者椎间盘以退变细胞为主 ,为退变早期阶段功能代偿期 ,脊髓型颈椎病椎间盘以坏死细胞为主 ,为退变晚期阶段 ,为不可逆期 ;
Objective: To observe the ultrastructure of the normal Cervical Disc Herniation(CDH) and Cervical Spondylotic Myelopathy (CSM) intervertebral discs and to study the morphology changes of degenerate human intervertebral disc.Methods Specimens were fixed at 4℃ and sent to make electron microscopic specimens immediately after being taken from 4 normal adults,2 patients with CDH and 8 patients with CSM.Results: No obvious difference was found in the collagen of the three groups.Cells in CDH discs showed repairing and compensatory signs.There were many lysosomes in them,which indicated the destructive function was elevated.Most cells in CSM discs demonstrated signs of necrosis.Conclusion:The CDH discs are composed mainly by degenerate cells that show active function,indicating that CDH is the early compensatory phase of degeneration.Most cells in CSM discs lose their functions,indicating that CSM is the late irreversible phase of degeneration.The degeneration morphology of cervical intervertebral discs is markedly different from that of the lumbar ones.
