摘要 目的 探讨乳腺癌中P53蛋白表达和微血管密度 (MVD)计数间的关系及其临床意义。方法 应用S P免疫组织化学方法检测乳腺癌中P53蛋白的表达并对微血管密度进行计数 ,同时对结果进行分析。结果 乳腺癌和癌旁不典型增生组织P53蛋白的阳性率分别为 3 9 6% (3 8/96)和 8.9% (6/5 6)。组织学Ⅲ级癌P53蛋白阳性率 (5 5 .3 % ,2 1 /3 8)高于Ⅰ级癌 (2 0 .8% ,5 /2 4) (P <0 .0 5 )。乳腺癌P53蛋白的阳性率与淋巴结转移和ER状态相关 (P <0 .0 5 ) )。 96例乳腺癌平均MVD(5 1 .3± 1 7.0 )明显高于癌旁不典型增生组织 (2 9.3± 9.5 ) (P <0 .0 1 )。MVD计数与组织学分级 (P <0 .0 1 )和淋巴结转移 (P <0 .0 1 )密切相关 ,但与ER状态无关 (P >0 .0 5 )。MVD计数与P53蛋白表达呈正相关 ,与 5年生存率呈负相关。结论 联合检测P53蛋白表达和MVD计数不但可以作为乳癌患者的预后指标 ,而且对揭示早期乳腺癌是有帮助的。 Objective To investigate the clinical significant of expression of P 53 protein and microvessel density (MVD) in breast carcinoma, and to evaluate correlation between them. Methods The expression of P 53 protein and FⅧRAg were measured by S-P immunohistochemical methods, mean while, MVD in the breast carcinoma were assay. Results The positive rate of P 53 protein in breast carcinoma and dysplasia of paracarcinomatons mucosa was 39.6%(38/96) and 8.9%(6/56), respectively. The positive rate of P 53 protein expression in histological grade Ⅲ (55.3%,21/38) was higher than that in grade Ⅰ (20.8%,5/24)(P<0.05). The positive rate of P 53 protein expression in breast carcinoma is associted with the lymph node metastasis and ER status (P<0.05). The mean of MVD in breast carcinoma (51.3±17.0) was markly higher than that in dysplasia of paracarcinomatous muuosa (29.3±9.5)(P<0.001). The MVD did not corrlate with ER status(P>0.05), but closely associated with the histological grade and lymph node metastasis (P<0.01,P<0.001). MVD was positively correlated to 5 year survival. Conclusion Combined expression of P 53 protein and MVD can be used as the prognostic markers for patients with breast cancer and may also be useful predictors of early malignant change in breast carcinoma.
