吗丁啉和铝碳酸镁合用对胆汁反流性胃炎疗效及胃内胆汁的影响

Effect of Domperidone Combine Hydrotalcid on Bile Reflux Gastritis and 24 Hour Intragastric Bile

目的:探讨促动力药吗丁啉和胆盐结合药铝碳酸镁对胆汁反流性胃炎的疗效及胃内２４小时胆汁的影响。方法:２５例胆汁反流性胃炎患者,经胃内２４小时胆汁监测证实,记录上腹痛、饱胀、恶心、呕吐症状的积分后,予以口服吗丁啉１０ｍｇ,每日餐前１５分钟服用,３次/ｄ,铝碳酸镁０．５ｇ,每日餐后６０分钟服用,３次/ｄ。治疗４周后观察上述症状积分的变化,并复查胃内２４小时胆汁反流情况。结果:治疗２周后４种症状积分较治疗前显著降低(Ｐ＜０．０５)。４周后腹痛、饱胀症状进一步改善,２４小时胃内胆汁反流总时间百分比由治疗前的３０．４±２０．２%降至１５．６±６．６%(Ｐ＜０．０１),反流次数由治疗前的１０４．９±５７．６次降至５０．８±１９．６次。结论:吗丁啉与铝碳酸镁合用能有效缓解胆汁反流性胃炎的症状,降低胃内胆汁反流指标,是治疗胆汁反流性胃炎的有效方案。

Purpose: To evaluate the effect of prokinetics, domperidone combine hydrotalcid on bile reflux gastritis and 24 hour intragastric bile consentration. Methods: Twenty five cases of bile reflux gastritis who were proven by 24 hour bile monitoring were admitted. The severity of the symptoms including abdominal pain, distention, nausea and vomiting bile were investigated before and after 2,4 weeks treetment by domperidone 10 mg three time one day combine with hydrotalcid 0.5 g three time one day. Intragastric 24 hour bile monitoring was also reexamined after 4 weeks treetment. Results; The severity of all the four symptoms greatly decreased after 2 weeks treetment(P<0.05). After 4 weeks treatment, the severity of abdominal pain and distention decreased furtherly( P < 0.01). The total fraction time of bile decreased from 30.4± 20.2 % to 15.6 ± 6.6 % (P < 0.01), and the number of bile reflux decreased from 104.9 ± 57.6 to 50.8±19.6(P <0.01). Conclusion: Domperidone combine hydrotalcid can release the symptoms of bile reflux gastritis and decreased the index of 24 hour intragastric bile monitering. Domperidone combine hydrotalcid is an effect treatment program for bile reflux gastritis.
