彩色多普勒超声对脾动脉栓塞术治疗前后的评价

Studied by Color Doppler Ultrasonography before and after Therapeutic Splenic Artery Embolization

目的 观察肝硬化门静脉高压患者和特发性血小板减少性紫癜 (ITP)患者行脾动脉栓塞术前后脾脏及脾静脉血流动力学变化。方法 应用彩色多普勒超声检查对 2 2例肝硬化和 12例ITP患者行脾动脉栓塞术前后的对比观察。结果 脾动脉栓塞术后两组患者脾静脉管径缩小 (P <0 .0 1) ,脾静脉平均血流速度降低 (P <0 .0 1) ,脾静脉血流量明显减少 (P <0 .0 1) ,脾动脉栓塞术后可见脾实质内梗塞坏死灶的回声。结论 本法可以定量分析脾动脉栓塞术前后脾循环的血流动力学改变 ,观查脾实质的二维影像对比 。

Objective To study the changes of the spleen and splenic venous hemodynamics in liver cirrhosis patients with portal hypertension and ITP before and after therapeutic splenic artery embolization. Methods 22 cases with ITP were observed and contrasted with color Doppler ultrasonography before and after therapeutic splenic artery embolization. Results After splenic artery embolization of the two groups, the diameterd of the splenic vein were reduced( P <0.01). TAV of the splenic vein was reduced ( P <0.01) and volume of venous returned were significantly reduced. And we could find the echo of infarction necrosis in spleen. Conclusion The way can quantitatively analysis the change of the splenic hemodynamics and observe the 2 D of spleen, and then get an effective evaluation to the splenic artery embolization.
