直肠吻合器在成人先天性巨结肠治疗中的应用及疗效观察

EEA STAPLER APPLICATION AND OBSERVATION IN TREATMENT OF HIRSCHSPRU-NG′S DISEASE IN ADULT

目的 :为改进成人先天性巨结肠的手术治疗方法 ,减少术后复发、避免术后二次闭瘘。方法 :回顾性分析了 32例(16～ 5 7岁 )成人先天性巨结肠患者采用一期低位直肠根治切除、吻合器吻合的临床资料。结果 :获随访的 19例患者在术后3～ 6个月 ,排便功能评分即达优良 ,12个月后达全优标准 ,12例男性患者无性功能障碍 ,全部病例无复发。结论 :采用一期根治性切除吻合器吻合是一种治疗成人先天性巨结肠的有效方法。该法减少了术后复发 ,避免了术后二次闭瘘 ,性功能及排便功能优良。

Objective:To improve the treatment of Hirschsprung's disease and decrease the postoperative complications such as recurrence and blind pouch,and avoid two stage revision of the stoma Methods:Retrospective analysis was made on 32 adult patients with Hirschsprung's disease treated with low anterior resection in one stage anastomosis with EEA staler(GF 1 type) Results:All patients were cured with satisfactory results,the followed 19 patients had good postoperative bowel function scores during 3 to 6 months,and all followed patients got excellent standard in a year,12 male patients had no sexual disability,and no patient has had recurrence by now Conclusions:EEA stapler application is an useful method in the treatment of adult Hirschsprung's disease It can eliminate postoperative recurrence,avoid two stage revision of the stoma with good sexual and bowel function
