经椎板间隙入路内窥镜下腰椎间盘摘除术

Discectomy under spinal endoscope via laminar interval approach

目的 探讨脊柱内窥镜下腰椎间盘摘除手术方法及临床效果。方法 经椎板间隙入路内窥镜下行腰椎间盘摘除和侧隐窝扩大术。术中用C型臂X线机定位、导引 ,显微内窥镜下切除纤维环和突出的椎间盘组织 ,咬除肥厚的黄韧带和增生内聚的关节突 ,扩大侧隐窝 ,对神经根进行彻底减压。结果 手术共 80例 ,无手术并发症 ,经平均 7个月随访 ,疗效优良率 98%。结论 应用微损伤脊柱内窥镜手术系统治疗腰椎间盘突出症具有损伤小、痛苦少、对腰椎稳定性影响小、适应症较广、术后恢复快、住院时间短等特点 。

Objectives To report the operative method and clinical efficacy of lumbar disk enudeation under spinal endoscopa.Methods Discectomy and lateral recess enlarged.operation via laminar interal approach under endoscope.After location and guide,it was complished by fluoroscopy, annalotomyed,discecfomyed, resected hypertrophic ligmentum.flarum and process articularis, enlarged lateral recess and decompressed nerve root under endoscope. Results Not a complication occured in all 80 operative case.The rate of excellent was 98% on average following-up for 7 months.Conclusions Using minimally injury spinal endoscope operative system to heal lumbar disc enudeation has the characters such as less injury,less instability, wide indications.fast recovery,shorter time in hospital,affirmative result.
