MRI动态增强在鉴别原发性骨肉瘤与瘤周水肿的价值(与病理标本对照) 被引量:8

摘要 目的 :评价MRI动态增强在鉴别骨肉瘤与瘤周水肿的价值。材料和方法 :本组收集15例标本完整用于病理取材与MRI动态增强对照研究 ,其中男性10例 ,女性5例 ,年龄11~54岁 ,平均年龄21.1岁。病变部位 :股骨下端7例 ,胫骨上端4例 ,肱骨上端2例及桡骨下端1例。结果 :本组15例中8例原发性骨肉瘤软组织肿块周围在MRI显示典型水肿信号 ,其表现为边缘不清、缺乏占位效应及信号均匀的羽样外观。水肿在T1WI呈低信号 ,T2WI及STIR呈高信号。MRI动态增强首次强化斜率值2.01 %作为判断肿瘤组织的标准 ,则MRI诊断的敏感性和特异性分别为84 %及86.6 %。结论 To evaluate the usefulness of dynamic contrast-enhanced MR imaging in differentiating between extraosseous extent and peripheral edema.Materials and Method:15 cases with primary osteosarcoma(male/female:10/5,age 11~54 years old,average 21.1 years old) were proved by operation and pathology.Position of lesions:femur 7,tibias 4,humerus 2,radius 1.Results:There was a typical MR imaging of peripheral edema in 8 patients.It presented a blurred border,no occupying effect and homogeneous signal feather's area.Signal of edema showed lower signal intensity on T1-weighted images,high intensity on T2-weighted images.Mean initial slope value of dynamic contrast-enhanced was 2.01%.Specificity and sensitivity of MRI were respectively 84% and 86.6%.Conclusion:Dynamic contrast-enhanced MRI is very valuable for differentiation between tumor and peripheral edema.
