剖宫产儿与母乳喂养的探讨

Effect of Breast Feeding on Newborn and Mother of Cesarean Section.

【目的】 探讨不同哺乳方式对剖宫产母婴的影响。 【方法】 随机将 83例剖宫产患者分为两组 ,研究组 42例剖宫产患者术后实行早吸吮 ,早接触 ,2 4小时母婴同室 ,纯母乳喂养 ,对照组 41例新生儿娩出后按手术产儿常规住婴儿室 ,3天内人工喂养不抱母乳 ,产后 72小时后每天抱喂母乳 5次 ,并辅助人工喂养。 【结果】 研究组产妇无 1例产后出血发生 ,产后 2 4小时内平均出血量较对照组少 12 5ml ,产后第 2天子宫复旧较对照组佳 ,下奶时间较对照组平均早 14小时 ,经统计学处理差异有显著性 (P <0 .0 0 5 )。研究组新生儿产后第 1、2天尿量少于对照组 ,提示新生儿有轻度脱水现象 ,但新生儿肺炎、腹泻、鹅口疮、臀红等疾病发病率及新生儿死亡率明显低于对照组 ,经统计学处理差异有显著性 (P <0 .0 0 5 )。 【结论】 在医护人员的帮助下对剖宫产产妇实行早吸吮 ,早接触 ,2 4小时母婴同室 ,坚持母乳喂养可促进子宫复旧 ,减少产后出血 ,提早产妇下奶时间 ,降低新生儿发病率及死亡率。

To investigate the effects of feeding patterns on mothers and newborns of cesarean section. Eighty three patients of cesarean section were divided into two groups atrandom,42 patients in study group and the others in control group. After operation in the study group early suck and contact were encouraged, mothers can keeptheir babies all the time,and newborns were give pure breast feeding.The newborns in control group were routinely put into newborn room after operation, provided with artificial feeding within 3 days. After 3 days, they were give breast feeding, supplemented with artificial feeding Mothers in study group did not have postpartum hemorrhage, the average volume of hemorrhage was 104ml less than that in control group in postpartum 24 hours, uterine recovery was better, and milk was excreted 14 hours earlier. The difference was significant( P <0.005). Newborns'urine volume of study group was less at first and second days, which indicated mild dehydration, while the morbidity of newborn pneumonia, diarrhea, thrush, redbuttocks and morbidity of newborn were significantly lower than that in controlgroup. Significant difference exists between the two groups( P <0.005). [Conclusions] Early suck and contact, staying withtheir babies and persistence in breast feeding can promote recovery of uterus, diminishpostpartum hemorrhage of mothers, promote early milk excretion, and decrease themorbidity and mortality of newborns.
