老年人垂体腺瘤及其显微外科治疗

Surgical treatment on pituitary adenoma in senile patients

目的 探讨老年人垂体腺瘤的临床特点及外科治疗。方法 回顾性分析了 46例 6 0岁以上垂体腺瘤病人的外科治疗。将手术方式和效果进行比较分析 ,同时分析比较了术后常规放疗和γ 刀立体定向放疗疗效。结果 全部病例均经外科手术 ,肿瘤全切 38例。大部分切除和次全切者行r 刀治疗 6例 ;随访 41例 ,平均随访 3.8年 ,临床效果满意。结论 应注意老年人垂体腺瘤常缺乏相应临床体征 ,易造成误诊和漏诊 ;对老年人垂体腺瘤的治疗以显微外科手术为主 ,减少并发症是降低术后病死率的重要措施和关键。

Objective To investigate the clinical characteristics and surgical treatment of pituitary adenoma in senile patients.Methods 46 cases (17 males,29 females) of pituitary adenoma treated with surgery were retrospectively analysed.The therapeutic effects of surgery,conventional radiotherapy and three dimensional γ knife targeted radiotherapy were compared.Results Radical resection were performed in 38 cases,and in 8 cases who had the major surgical resection and sub radical resection 6 were treated by γ knife.41 cases were followed up from 0.3 to 9.2 years with an average of 3.8 years and good therapeutic effects were obtained in all of the patients.Conclusion Attention should be paid to the clinical characteristics which are rarely manifested in senile patients with ptiuitary adenoma and microsurgical operation is the first choice of treatment. Reducing the postoperative complications plays an important role in lowering mortality.
