术中放疗配合根治术治疗Ⅱb期宫颈癌

Intraoperative radiation therapy assisted with pelvic radical resection to treat cervical carcinoma stage Ⅱb.

目的 :探讨术中放疗的实施及对Ⅱb期宫颈癌术中放疗的临床价值。方法 :Ⅱb期宫颈癌患者先体外全盆照射加根治术 ,再术中放疗。盆腔平面照射总剂量为 4 5Gy ,术中一次照射剂量为 17 5～ 2 0Gy。结果 :术中放疗手术时间仅比常规手术延长约 2 0min ,近期无严重并发症发生 ,切口愈合良好 ,不增加住院天数 ,也不影响其他治疗 ,近期疗效满意。结论 :术中放疗可提高肿瘤的局部照射剂量 ,作为一种放疗加量的手段 。

Objective:To investigate the application of intraoperative radiation therapy (IORT)and its clinical value in cervical carcinoma stage Ⅱb.Methods:All patients were given a comprehensive treatment including preoperative external wholepelvic radiation ,radical hysterectomy,pelvic lymphadenectomy and intraoperative radiotherapy.The total external radiation dosage in central pelvis was about 45Gy. For single time radiation,the dose was about 17.5～20Gy intra in operation. Results: The operation time prolonged for only 20 minutes in IORT than that in the routine operation.There were no severe complication in the near future,no adding of the in hospital days,and no effect to the other therapy.The incision was well healing.The treatment outcome in near future was satisfying. Conclusions:IORT can increase the local target radiation dosage.It may be applicable in gynecological malignant tumor as a way to increase the radiation dose.
