清热祛湿法对兔痛风性关节炎细胞因子的影响

Exprimental study about IL-1β IL-8′s Change on the Treatment on GOUT with TCM of Releasing Heat And Eliminating Dampness

目的 :通过观察并分析清热祛湿中药对兔膝急性痛风性关节炎的膝关节液中白介素 1β、白介素 8(IL 1β、IL 8)的影响 ,并与西药秋水仙碱对比研究 ,探讨清热祛湿中药治疗急性痛风性关节炎的机理 ,来证实湿热痹阻是急性痛风性关节炎的主要中医病机。方法 :将 48只大白耳兔随机分为 4组 ,采用McCartyD J经典方法向兔膝关节注入尿酸钠 (MSU)溶液造成急性痛风性关节炎的病变模型 ,以清热祛湿中药与秋水仙碱对比治疗。于造摸后第 2h、6h、9h、12h分别取材 ,采用双抗体夹心酶联免疫吸附实验 (ELISA)法 ,分别测定各组、各期兔膝关节液中IL 1β、IL 8的水平。结果 :造模组IL 1β、IL 8的水平明显高于中药组和西药组 ,有显著差异 ;空白对照组未能测出IL 1β、IL 8;中药组IL 1β、IL 8的水平低于西药组 ,有统计学差异。结论 :①IL 1β、IL 8在急性痛风性关节炎的发病过程中可能起重要作用 ;②清热祛湿法中药能有效降低实验性痛风兔膝关节液的IL 1β、IL 8水平 ,其效果优于西药组 ;③湿热痹阻可能是急性痛风性关节炎的主要病机 ,清热除湿法是治疗急性痛风性关节炎的有效方法。

Objective :By analyzing the change of TCM of releasing heat and eliminating dampness and colcemide on the level of IL-1βand IL-8 in knee-joint fluid of rabbit′s gout,discussing the mechanism of TCM of releasing heat and eliminating dampness on treating gout and proving that dampness and heat are the main mechanism of acute urathritis. Method: 48 rabbits,were divide into 4 groups randomly,by injecting MUS into knee-joint gout constrasting treatment with TCM of realeasing heat and eliminating dampness and colcemide.To detect the level of IL-βand IL-8 knee-joint fluid of eyery group and at(2h,6h,9h,12h) by ELISA. Result :1.The level of IL-1β and IL-8 of model group was significantly highter than that of TCM group and colcemide group,( P <0 01).IL-1β and IL-8 of contral group were not detected.2.The level of IL-1βand IL-8 of TCM group was lower than colcemide group( P <0 05). Conclusion :1.Il-8 and IL-1β had important function on the course of acute urarthritis.2.TCM of realeasing heat and eliminating dampness could sufficiently depress the level of Il-1β and IL-8 in knee-joint fluid of experimental urarthritis rabbits.3.The effect was better than colcemide.it proved that abstruct due to dampness and heat was probably the main mechanism of acute urthritis,and releasing heat and eliminating dampness was the effective method treating acute urarthritis.
