新生儿败血症病原菌及其耐药分析 被引量:9

ANALYSIS ON THE PATHOGENIC BACTERIA AND RESISTANCE TO ANTIBIOTIC OF NEONATAL SEPTICEMIA

摘要 了解近期我院新生儿科败血症主要病原菌及其耐药情况 ,以期减少临床上应用抗生素的盲目性 ,降低耐药菌的产生。对 1 998年 6月 1 999年 1 2月入住我院新生儿科临床诊断败血症的 70例患儿 ,总共血培养出细菌 77株。用VITEK AMS鉴定菌株种属 ,K B法作抗生素耐药试验 ,根据NCCLS标准判断结果。凝固酶阴性葡萄球菌 ( 39株 )及金黄色葡萄球菌 ( 2 1株 )为主要致病菌 ,几乎全部对青霉素耐药 ,对红霉素及苯唑青霉素耐药率均超过 75% ,对万古霉素及环丙氟哌酸高度敏感。尚未发现耐万古霉素的葡萄球菌株及肠球菌株。几种杆菌 (大肠埃希菌 6株、肺炎克雷伯杆菌3株等 )对氨苄青霉素及庆大霉素耐药 ,对阿米卡星及第三代头孢霉素敏感。未测出ESBLs。本组资料显示绝大部分病例为非院内感染。 To understand the pathogenic bacteria and resistance to antibiotic of neonatal septicemia in department of neonate of our hospital recently, it may reduce clinical blindness of antibiotic use and decrease resistant-bacteria. From Jul. 1998 to Dec. 1999, in the neonates that have been diagnosed septicemia 70 blood-culture are positive. 77 bacteria have been obtained. VITEK-AMS has been differentiated. Plasma-coagulase negative Staphylococcus 39, S.aureus 21, Escherichia coli 6, Klebsiella pneumoniae 3, Streptococcus 3, Enterococcus faecium 2, the other 3. Isolates were tested for susceptibility to antibiotic by means of the disk-diffusion method of B-K, following NCCLS recommenedations. Almost all Staphylococci resist PEN, 75% of them resist ERY and OXA, highly susceptible to VAN and CIP. Staphylococcus and Enterococcus of resistance-VAN have been not finded. Bacillus resist AMP and GEN, susceptible to the thirdgeneration Cephalosporins. ESBLs has been not examined.
