摘要 目的 利用早产儿尸检的整块股骨 ,研究早产儿时期骨形成和吸收静态参数的变化 ,探讨早产儿代谢性骨病的发病机制 .方法 出生后 1wk内死亡的早产儿 2 1(男 12 ,女9)例 ,胎龄 2 5~ 36 wk,出生体质量 484~ 2 44 5 g,应用半自动计算机技术对骨皮质、骨小梁进行形态学测量 .结果 随着胎龄的增加 ,骨容积 (BV/ TV)明显增加 ,骨表面 (BS/ BV) ,类骨质容积 (OV/ BV)和静止表面 (RS/ BS)明显减少 ;男性较女性的骨皮质形成参数 OV/ BV,Ob.S/ BS,O.Th和骨吸收参数 N.Oc/ TAr明显增高 ;随着体质量的增加 ,Tb.Th,Vd.Th明显增加 .结论 早产儿骨吸收增加 ,骨形成减少 。 AIM To study parame ters of bone formation and resorption,as well as the pathogenesis of metabolic bone disease in premature infants. METHODS Using t he semiautomatic computerized technique to performed histomorphometric analysis of trabecula and cortex of wh ole femora specimens. RESULTS With the growth of the fetus, the bone volum e (BV/TV) increased significantly while the bone surface (BS/BV), osteoid volume (OV/BV), and rest surface (RS/BS) decreased significantly; In both female and male fetuses, no significant change was defected in the parameter of bone formation and resorption of trabecular, b ut the parameters of bone formation of cortex including OV/BV Ob.s/BS O.Th and the parameters of bone resorption osteoclasts number (N.Oc/T.Ar) were significantly higher in the male group than in the female group. The trabetcular thickness (Tb.Th) and Void thickness (Vd.Th) increased significantly with increasing birthweight. CONCLUSION Pathogenesis of metabolic bone disease might increas e bone resorption and decreasing bone formation in premature infants.
