老年心绞痛患者细胞粘附分子测定及调脂治疗观察

Detection of serum cellular adhesive molecule and intervention of lipid regulation in elderly patients with angina pectoris

目的 探讨老年心绞痛患者冠状动脉粥样病变炎症反应及调脂干预对其产生的影响。 方法 测定 60例老年心绞痛患者和 3 0例健康老人的血清P 选择素、氧化修饰低密度脂蛋白 (OX LDL)、一氧化氮 (NO)水平并对病例组给予辛伐他汀调脂治疗。 结果 老年心绞痛组血清P 选择素 (1 92± 0 88)mg·L-1、OX LDL(1 5 4±0 5 6)mg·L-1比对照组 (0 90± 0 3 2 )mg·L-1、(0 76± 0 15 )mg·L-1明显增高 ,NO水平老年心绞痛组为 (68 5 2± 13 41)mmol·L-1,对照组 (119 88± 2 6 85 )mmol·L-1,明显降低 (P <0 0 5～ 0 0 1) ;相关分析发现P 选择素与OX LDL呈显著正相关 (r =0 65 ,P <0 0 1) ,与NO呈显著负相关 (r =-0 72 ,P <0 0 1)。调脂治疗后病例组血清P 选择素、OX LDL水平明显降低 (1 2 6± 0 3 8)mg·L-1、(0 92± 0 3 3 )mg·L-1,P <0 0 1,NO水平明显回升 (113 2 6± 2 7 75 )mmol·L-1,P<0 0 1。 结论 老年心绞痛患者存在冠状动脉粥样病变炎症反应 。

Objective To explore the inflamation in the coronary arteriosclerotic region and the effects of intervention of lipid regulation in elderly patients with angina pectoris. Methods The levels of serum P selectin,oxidized low density lipoprotein (OX LDL) and nitric oxide(NO)were measured in 60 elderly patients with angina pectoris and 30 normal controls. Results The levels of serum P selectin (1 92±0 88)mg·L -1 and OX LDL(1 54±0 56)mg·L -1 were higer and the level of serum NO(68 52±13 41)mmol·L -1 was lower in group of patients than that in normal controls(0 90±0 32)mg·L -1 ,(0 76±0 15)mg·L -1 ,(119 88±26 85)mmol·L -1 .P selectin was positively correlated with OX LDL ( r =0 65, P <0 01)and negatively correlated with NO( r =-0 72, P <0 01).The serum P selectin and OX LDL were reduced (1 26±0 38)mg·L -1 ,(0 92±0 33)mg·L -1 and serum NO was elevated [(113 26±27 75)mmol·L -1 , P <0 01]by the intervention of lipid regulation in the group of patients. Conclusions The elderly patients with angina pectoris had inflammatory infiltration in coronary atherosclerosis and it was blocked by the intervention of lipid regulation.
