急性颈髓损伤的高场强MRI信号特征及临床意义

Characteristic Appearances of Acute Spinal Cord Injury in MRI Correlated with the Deficit of Neural-Function

目的 :探讨急性颈髓损伤后高场强MRI特征性表现 ,并分析其与临床神经功能相关性。材料和方法 :对54例急性颈髓损伤患者ASIA分级后行高场强MR成像 ,分析创区颈髓不同信号特点与神经功能缺失的相关性。结果 :28例颈髓损伤于T2WI表现为“环征”信号特点 ,其临床表现为脊髓完全损伤 ;26例MRI表现为长T1 长T2 信号变化 ,临床表现为脊髓不完全损伤 ,其损伤的长度反映出不同神经功能缺失 ,长度大于2.39±0.36cm,其神经功能缺失严重 ,长度小于0.84±0.05cm ,神经功能损伤较轻。结论 :高场强MRI所表现不同信号特征可反映不同临床神经功能。

Purpose:To study the correlation between characteristic findings in MRI and loss of neural-function in acute spinal cord injury(ASCI).Materials and Method:54 patients of ASCI were grouped according to ASIA classification and then were performed with MRI scan.Results:28 patients showing signs of complete cord injury were found to have 'circle-around' sign on T2WI,26 patients with signs of incomplete injury were found to show hypo and hyper intensity on T1/T2WI;the extent of injuried spinal cord reflected the loss of neural-function,which was more severe when the length of the injured cord was over 2.39±0.36cm,while a value less than 0.84±0.05cm suggested a mild lesion.Conclusion:MRI signal features of ASCI are closely correlated with neural-function.
