系统性红斑狼疮中股骨头缺血性坏死的临床分析

The Clinical Analysis for Avascular Necrosis of the Femoral Head in Systemic Lupus Erythematosus

目的 :探讨系统性红斑狼疮中股骨头缺血性坏死的发病机理、早期诊断、治疗及预防。方法 :总结系统性红斑狼疮 30例 ,全部接受激素治疗 ,定期检测血钙、血液粘度等。结果 :发现 4例患者在不同病程、不同激素用量的情况下 ,X线示股骨头缺血性坏死。结论 :系统性红斑狼疮中股骨头缺血性坏死的发生与激素的蓄积量及短期内激素追加量有关。与血液粘度增高、血钙下降有关联。早期诊断 ,适时行股骨头假体置换术可提高患者的生活质量。

Objective: To study the etiopathology,early diagnosis,treatment and prevention of avascular necrosis of the femoral head in systemic lupus erythematosus. Methods: 30 cases of systemic lupus erythematosus were summarized, all of them received corticosteroid therapy with regularly serum calcium,blood density check up and so on. Results: In different courses of the disease different quantities of corticosteroid, 4 cases had X-ray finding of avascular necrosis in the femoral head. Conclusion: The occurrence of avascular necrosis of the femoral head in systemic lupus erythematosus has related to the quantity of storing up corticosteroid and its increment quantity in a short time. increment of blood density and decrease of serum calcium. Early diagnosis and operation at the right moment can improve the patient's quality of life.
