肺癌^(18)F—FDG和^(99m)Tc-MIBI显像研究

摘要 目的 探讨^(18)F-FDG和^(99m)Tc-MIBI两种显像方法对肺癌诊断价值。方法 患者禁食4小时以上,静脉注入37MBq^(18)F-FDG,静卧45分钟后进行FDG显像。MIBI显像方法是早餐吃油煎鸡蛋喝牛奶,静脉注入1.11GBq^(99m)Tc-MIBI,之后60分钟进行胸部断层显像检查,32例肺癌患者加作120分钟延迟显像。结果 41例FDG显像的阳性率是92.7%(38/41),靶本比2.90±1.35。41例MIBI 60分钟显像的阳性率是75.6%(31/41),靶本比1.92±0.59;32例延迟显像的阳性率为78.1%(25/32),靶本比1.78±0.53。FDG的靶本比和阳性率高于MIBI(P<0.05),鳞癌和腺癌FDG显像的靶本比无差异(P>0.05)。结论 FDG的显像质量和阳性率均优于MIBI,MIBI阴性显像结果是否与多药耐药有关有待进一步研究。 Purpose To study the diagnostic value of 18F - FDG and 99mTc - MIBI imaging of lung cancer. Methods After fasting for 4 hours, the patient was given a dose of 37MBq 18F - FDG i. v. and the FDG imaging was performed 45 minutes after injection. Before MIBI scan, the patient was told to eat pan - broiled breakfast and a doses of 1.11GBq 99mTc - MIBI were injected i. v. After 60 minutes the scintigraphy was performed, additionally, 32 patients were given delayed scans after 120 minutes. Results The abnormal positive results of FDG imaging was 92.7% (387 41) and the tumor - to - normal (T/N) ratio was 2.90 ± 1.35. Whereas, after 60 minutes of injection the positive results of MIBI imaging was 75. 6 % (31/41) and the T/N ratio was 1.92 ± 0.59. After 120 minutes the positive rate and the T/N ratio of 32 delayed MIBI imagines were 78.1 % (25/32) and 1.78 ± 0.53 individually. The T/N ratio and positive results of the FDG imaging were more than MIBI ( P < 0.05 ) . The T/N values between the squamous cell carcinoma and adenocarcinoma had no temarkable difference (P > 0.05). Conclusion The image quality and positive rate of FDG is superior to 99mTc - MIBI, whether the negative MIBI imaging had correlation with multidrug resistance remained to be illustrated.
