显微手术治疗颅底肿瘤86例临床分析

The clinical analysis of 86 cases of basicranial tumours treated by microsurgery

目的 探讨颅底肿瘤的显微手术治疗方法。方法 总结 86例颅底肿瘤的临床表现、影像学特征及显微手术治疗的经验。结果 肿瘤全切除 69例 ,次全切除 11例 ,部分切除 4例 ,死亡 2例 ;术前颅神经功能损伤 64例 72条 ,术后恢复 3 8条 ,好转 11条 ,症状无改善 14条 ,加重 9条 ,新颅神经损伤 7条。结论 显微手术切除颅底肿瘤是更为安全和有效的治疗方法 ;熟练的显微外科技术和技巧 ,是提高肿瘤全切除率 。

Objective To probe into the treatment of basicranial tumors by microsurgery.Methods The clinical manifestation,image characteristics and the microsurgery experience of 86 cases of basicranial tumours were summed up.Results The total,sub total,and partial resection were carried out in 69,11 and 4 cases respectively.2 cases died,Among 64 cases 72 pieces of cranial nerves were injured before operation,the function of injured cranial nerves recovered in 38 pieces,bettered in 11 pieces,hardly improved in 14 pieces and worsened in 9 pieces after operation,and the new injury of cranial nerves occured in 7 pieces.Conclusion Microsurgery was a safer and more effective approach to resect the basicranial tumors,skilled microsurgery techniques,familiar with basicranial micro anatomy are the key to enhancing the total tumor resection rate,reduce the operation death rate and the injury of cranial nerves.
