先天性青光眼外路小梁切开术的临床观察

CLINICAL OBSERVATION ON TRABECULOTOMY OF CONGENITAL GLAUCOMA

作者对45例72只眼行Luntz法外路小梁切开术,41例66只婴幼儿性青光眼的早期手术成功率为92%,对其中32例50只眼随访3～36个月,一次手术成功率为79%,二次手术成功率为82%,对4只眼加用药物,眼压控制可达90%。还对4例6只青少年性青光眼行了手术,取得一定效果。

This article reported Luntz′s trabeculotomy by external route on 45 cases 72 eyes,among those 41 cases 66 eyes were infantile glaucoma, achieving an early surgical success rate of 92%.32 cases 50 eyes were fol- lowed up to 3—36 months.The average follow-up duration was 18 months. Success rate with one surgical operation was 79%.Success rate with two surgical operations was 82%.IOP of 90% eyes were controlled with a com- plement of drugs on 4 eyes.A detailed Luntz′s trabeculotomy by external route was introduced.A sac-based conjunctival flap was prepared.Direct minor incision of 3 mm was made on the sclera posterior to the limbus. Surgical markings could easily be distinguished.No visible scarrings in the surgical field.It is believed this operation is more easily conducted than other trabeculotomies.Causing less damage to the tissues,it can be repeated. And it is a valuable operation of choice to be popularized for the treatment of congenital glaucoma.
