惊厥并发低钠血症的因素

The Factors in Convulsion Complicated by Hyponatremia

目的 :探讨惊厥合并低钠血症的各种因素及发病机制 ,采用回顾分析法。方法 :对 168例有惊厥发作的住院患儿进行统计学分析。按血钠值分为 :低钠组、临界组及正常对照组。结果 :不恰当治疗分别占低钠、临界和正常对照组的 12 /33( 36 4% )、9/14 ( 2 2 0 % )、8/89( 9 0 % ) ,前 2组分别与对照组比较有显著性差异 (P <0 0 1,P <0 0 5 )。结论 :不恰当补液及脱水治疗为低钠血症的重要发病因素 ,缺氧、外伤、炎症为相关因素 ,血脑屏障受破坏 ,血氧分压下降及抗利尿激素 (ADH)分泌失控 ,为其可能发病机制。

Objective To explore the pathogenesis and the factors related to convulsion complicated by hyponatremia.Methods Clinical data was collected in 168 convulsion in patients by retrospective statistical analysis.Results Improper therapy was carried out in 12(36.4 %),9(22.0 %),8(9.0 %) respectively in the pyponatremia,the critical and the control group which were assigned with sodium levels in blood.There were significant differences between the control group and the others( P <0.01, P <0.05).Conclusion The main cause of hyponatremia is incorrect fluid supplemented and diuresis.Hypoxia,inflammation,and trauma to the head were correleted with hyponatremia.The potential pathogenesis is that the blood brain barrier is ruined,partial pressure of oxygen(PO 2) depleted and supersecretion of antidiuretic hormone (ADH).
