平板运动试验对冠状动脉狭窄的诊断价值

摘要 目的 :探讨平板运动试验对冠状动脉 (冠脉 )狭窄程度及范围的诊断价值。方法 :98例胸痛患者平板运动试验结果与冠脉造影结果作对照分析。结果 :平板运动试验诊断冠心病的敏感性为 83.1% (4 9 5 9) ,特异性为 6 9.2 % (2 7 39) ,准确性为 77.6 % (76 98)。平板运动试验中ST段压低幅度越大 ,持续时间越长 ,运动中收缩期血压增量越小 ,冠状动脉狭窄程度就越重。试验中ST段压低幅度越大 ,持续时间越长 ,出现ST段压低的导联数目越多 ,运动中最大心率越小 ,冠脉狭窄范围就越广泛。结论 :平板运动试验可初步估测冠脉狭窄程度及狭窄范围 ,不失为冠心病诊断中又一有效的无创性检查方法。 Objective:To study the value of treadmill exercise test(TET) in evaluating the severity of coronary artery stenosis.Methods:The results of TET were analyzed in 98 suspected patients with coronary artery disease. All patients have undergone coronary angiography(CAG).Results:The comparative study showed that the sensitivity of TET was 83.1%,specificity 69.2% and accuracy 77.6%. The deeper ST depression,the longer duration of ST depression and more lower the systolic blood pressure(SBP) lower during TET in patients,the more severe the stenosis of coronary artery. Furthermore,the larger number of the lead with ST depression larger and the slower the maximum heart rate slower during TET in patients the more extensive stenosis of coronary artery.Conclusion:TET can evaluate the severity and the range of coronary artery stenosis and may be used as another diagnostic aid.
