AFP、GPDA-F与GGT-Ⅱ对原发性肝癌的互补诊断 被引量:7

摘要 目的 探讨联合检测甲胎蛋白(AFP)、甘氨酰脯氨酸二肽氨基肽酶同工酶F(GPDA-F)和γ-谷氨酞转肽酶同工酶Ⅱ(GGT-Ⅱ)对原发性肝癌的互补诊断价值。方法 对101例肝癌和124例其他肝病患者血清AFP、GPDAF和GGT-Ⅱ进行了同步检测,对比分析联合检测这三项指标对肝癌的互补诊断价值。结果AFP、GPDA-F和GGT-Ⅱ诊断肝癌的敏感性分别为 72.3%、85.1%和 66.3%;联合检测 GPDA-F和 GGT-Ⅱ或 AFP可使敏感性提高至 92.1%和 93.1%;三项指标联合检测使肝癌的检出率高达97.0%。结论AFP、GPDA-F和GGT-Ⅱ之间存在有互补诊断价值,联合检测这三项标志物可提高肝癌尤其是AFP阴性肝癌的诊断效率。 Objective To study the complementary value of alpha-fetoprotein (AFP), glycylproline dipeptidyl aminopeptidase isoenzyme F (GPDA-F) and gamma-glutamyltransferase isoenzyme II (GGT-II) to the diagnosis of primary hepatic cancer. Meth-ods Serum AFP, GPDA-F and GGT-II were measured inlOlcases with primary hepatic cancer and 124 cases with other liver dis-eases. The three indices were compared and analyzed for the their complementary diagnostic values to primary hepatic cancer. Results The diagnostic sensitivities of AFP, GPDA-F and GGT-II to primary hepatic cancer were 72.3% , 85.1 % and 66.3% respectively. Simultaneous determination of GPDA-F and GGT-II or AFP could increase the sensitivities to 92.1 % and 93.1 % re-spectively. Furthermore, combined detection of these three indices could make the diagnostic sensitivity as high as 97.0% . Conclu-sion AFP, GPDA-F and GGT-II are complementary for the diagnosis of primary hepatic cancer. Simultaneous determination of these three markers may increase the diagoostic efficiency to primary hepatic cancer especially for the cases with negative AFP.
