P21^[WAF1]阳性乳腺癌患者的预后观察

Prognoctic investigation on P21[WAF1] positive human breast cancer

目的探讨 P21[WAF1]在乳腺癌中的表达与预后的关系。方法以免疫组化 S－ P法检测 152例乳腺癌组织 P21[WAF1]基因蛋白的表达水平。结果 152例中 P21[WAF1]蛋白阳性 61例，占 40. 1％；其中低表达 43例，占 70. 5％，高表达 18例，占 29. 5％，两者差异有显著性 (P < 0. 01)。 P21[WAF1]的表达水平与组织学分级相关。单因素生存分析显示 P21[WAF1]蛋白低表达组优于表达组 (P < 0. 01)。 Cox模型多因素分析显示 P21[WAF1]蛋白表达是影响预后的独立指标。结论 P21[WAF1]在乳腺癌中不仅起着抑制癌细胞生长的负向调节作用，而且还可能存在着促进癌细胞增殖的正向调节功能。

Objective： To investigate the relationship between P21[WAF1] expression at level to breast cancer prognosis.Methods:P21[WAF1] expression at protein level was determined by S－ P immunohistochemistry at protein in 152 patients with breast cancer.Results:Of the 152 paticent ,P21[WAF1] expression was low in 43(70.5％ )and high in (18(29.5％ ),respectively,P21[WAF1] expression level was correlated with the tissue grade of breast cancer.Univariate analysis demonstrated disease－ free survival in patients with lower P21[WAF1]expression than in those with higher expression,bowrver,by Cox multivariate analysis,P21[WAF1] expression level was not found to be independem prognostic signiticance.Conclusion: P21[WAF1]not only control the cell of breast cancer in the negative regulation,but also possibly urge the cell of breast cancer growth in positive regulation.
