重型颅脑损伤后脑沟脑池改变与颅内压及预后的关系

Relationship between the changes in cortical sulcus or cistern and intracranial pressure and prognosis in patients with severe head injury

目的：探讨重型颅脑损伤（ＳＨＩ）病人脑沟脑池形态改变与颅内压（ＩＣＰ）和预后的关系。方法：对１２０例ＳＨＩ（ＧＣＳ≤８分）病人ＣＴ片上脑沟脑池的形态改变、ＩＣＰ及死亡率（ＭＲ）作统计分析。结果：轻度ＩＣＰ增高表现为脑沟及外侧裂池消失；中度及重度ＩＣＰ增高表现为鞍上池及环池消失。鞍上池及环池消失的病人ＭＲ较高。结论：根据ＳＨＩ病人ＣＴ片上脑沟脑池的形态改变可判断ＩＣＰ的高低及病人的预后。

Objective: To explore the relationship between the changes on cortical sulcus or cistern and intracranial pressure and prognosis in patients with severe head injury. Methods: 120 cases with severe head injury(GCS≤ 8) were Statistically analyzed,including CI' image,intracranial pressure and mortality rate. Results: Mildly increased in ICP resulted in cortical sulcus effacement or lateral fissure cistern effacement. Highly increased in ICP resulted in supracellar cistern or ambient cistern effacement. Patients with ambient cistern effacement have the highest mortality rate among this group. Conclusion: According to the CF image,ICP and prognosis can rounghly be estimated.
