超声多普勒估测肺动脉压力的评价

摘要 本文采用脉冲多普勒超声心动图(PDE)技术,测定27例左向右分流型先心病患者右室收缩时间期(RVSTI)的右室射血前期(RVPEP)、右室射血时间(RVET)、右室射血加速时间(RVAT)3个单项指标及由其两两相比得到的6项比值共9项指标,与心导管所测主肺动脉压力(PAP)各指标(PASP、PAMP、PADP)相对照,以探讨用PDE定量测PAP的价值。结果表明;PDE所测RVSTI值以估测PAP不仅简便可行,而且准确可靠,具有较高临床实用价值。 27 cases of congenital heart disease with left-to-right shunt were studied.The PAP of these case were determined by PDE in three parameters of RVSTI; they were RVPEP,RVET,RVAT and the six ratios: RVPEP/RVET, RVPEP/RVAT, RVET/RVPEP, RVAT/RVET, RVAT/RVPEP, RVET/RVAT; the above nine parameters were calculated statistically and were compared with the simultaneous PAP by the angiocardial catheterization (cath)in the results of PASA, PAMP, and PADP. In these studies, the best parameter by PDE was RVPEP/RVAT which were well correlated with the values of PAP by cath (γ=0.81, 0.87,0.86), the next better ratio was RVPEP/RVET. By statistics with stepwise regression analysis the coefficient of correlation(γ) elevated to0.88, 0.94, 0.95 between the results by PDE and cath. The specialty and sensitivity of determining PAP by PDE were 83% and 80% respectively. Measuring the RVSTI by PDE to determine the PAP quantirely were practical, reliable and easy to do clinically. The result of this study was valuable to be reeommened.
