摘要 目的 对 5 4例 74眼高度近视白内障手术进行分析 ,74眼眼轴均≥ 2 6 mm ,最长 34 .48m m,探讨其手术的安全性、有效性。方法 囊内摘出 2眼 ,现代囊外摘出 2 7眼 , 期植入后房型人工晶状体 45眼 ,手术方法同常规白内障 ,采用球后加面神经阻滞麻醉。结果 术后视力 <0 .1者 5眼 ,0 .1~ 0 .2者 2 6眼 ,0 .3~ 0 .5者 2 0眼 ,0 .6~ 0 .9者 19眼 ,≥ 1.0者 4眼。 1例 2眼术后出现视网膜脱离。结论 高度近视眼白内障手术安全、有效、并发症少 ,眼底病变及眼轴长度与术后视力关系明显 ,人工晶状体植入后视力明显好于非植入组。 Objectives To analyze the results of cataract surgery of 74 eyes in 54 cataract cases with high myopia,whose axial lengths were>26mm and the longest was 34.48mm and to discuss the safety and value of surgery.Methods ICCE in 2 eyes,ECCE in 27 eyes,ECCE with posterior chamber IOL implantation in 45 eyes.The surgical procedures were identical with routine cataract surgery.Retro bulbal and facial block anesthesia were performed.Results Visual acuity was <0.1 in 5 eyes,0.1~0.2 in 26 eyes,0.3~0.5 in 20 eyes,0.6~0.9 in 19 eyes,≥1.0 in 4 eyes,and retinal detachment in 2 eyes on 1 case. Conclusions Cataract surgery in high myopia is safe and desirable.The operative complications are seldom seen.Pathologic changes in the fundus and axial length are in close contact with visual acuity.Visual acuity of eyes with IOL implantation is better than without IOL implantation.
