慢性前列腺炎诊治及疗效评价(附600例报告)

Chronic prostatitis:diagnosis,treatment and curative effect of 600 patients

目的 探讨慢性前列腺炎的诊疗标准。 方法 调查 6 0 0例慢性前列腺炎病例的实验室诊断结果 ,按照美国国立卫生研究所 (NIH)制定的前列腺炎症状评分 (CPSI)标准 ,进行NIH分类和疗效的判定。应用喹诺酮类抗生素 4～ 12周 ,根据病情适当应用α受体阻滞剂、热疗或前列腺按摩疗法。 结果 按NIH分类 ,本组Ⅱ型 80例 (13 .3% ) ,ⅢA型 335例 (5 5 .8% ) ,ⅢB型 185例(30 .9% )。治疗 6周前后CPSI症状评分判定 :临床治愈 138例 (2 3% ) ,显效 (CPSI减少 >15 ) 372例(6 2 % ) ,有效 (CPSI减少 <15 ) 6 0例 (10 % )。无效 30例 (5 % )。 结论 两杯判定试验 (PPMT法 )进行前列腺炎的诊断分类简单易行。CPSI是一种较好的疗效评价方法。喹诺酮类抗生素、α受体阻滞剂和止痛剂等药物的联合应用对慢性前列腺炎具有较好疗效。

Objective To study several critical problems in the diagnosis and treatment of chronic prostatitis. Methods Based on National Institute Health (NIH) classification,600 cases of chronic prostatitis were surveyed on laboratory test and clinical history.In addition,chronic prostatitis symptom index (CPSI) of each patient was evaluated.The antimicrobials such as quinolone were administered for 4 to 8 weeks,and the alpha blockade,anti-inflammatories,thermotherapy and prostatic massage were given to part of the patients. Results According to NIH,80 cases were classified to be type Ⅱ,335 were type ⅢA and 185 were type ⅢB.From the results of CPSI pre and post 6 weeks treament,the curative effect was significant.The cure rate was 23%(138 cases),the symptom of 372 patients (62%) were improved significantly,whose CPSI decreased more than 15,and only 60 patients (10%) felt a little better,whose CPSI decreased from 5 to 15. Conclusions PPMT is a reliable method to diagnose prostatitis,and easy to carry out.NIH-CPSI may play an important role in evaluating the curative effect.The triple drugs,including quinolone,Alpha blockade and anti-inflammatories,will be an effective therapy for chronic prostatitis.
