内镜下经口底入路行颌下腺切除术

ENDOSCOPIC RESECTION OF SUBMANDIBULAR GLAND THROUGH: MOUTH FLOOR APPROACH

目的 探讨颌下腺切除的新术式,比较以往单纯从口外颌下皮肤切口的术式。方法 于经鼻全麻下,切口采用口内入路长约3cm,自第一双尖牙近中至第二磨牙远中舌侧利用窥镜口底入路,分别切断与颌下腺有关的神经、动脉、静脉及导管,完整摘除颌下腺。结果 4例患者均顺利切除颌下腺,术后无并发症。结论 内窥镜切除颌下腺在临床上是可行的,颌面部皮肤无手术切口,利于美观,同时此术式有效避免了面神经下颌缘支的损伤。

ve To evaluate the procedure of endoscopic resection of submandibular glands, in past, surgeons usually resected the submandibular glands through the skin of submandibular area. Methods Under general aneasthesia through nasal cavity, we used the endoscope to resect submandibular glands through the floor of mouth. Length of incision is about 3 cm from lingual of mesial frist bicuspid to distal Second molar. Using endoscope, we dissected the arterry、ein 、nerve and duct associated with submandibular glands. Then cut up them. The submandibular glands had removed off completely . Results All 4 patients's submandibular gland were safely resected. There are no postoperative symptoms and syndromes . The incision is more secluded than the traditional operation . More important, this technique can effectively avoid the injury of marginal mandibular branch of facial nerve. Conclusion This technical innovation is safe, accurate and effective .
