摘要
目的 :通过对非高角型和高角型骨性安氏 类 1分类错牙合的矫治 ,找出各自牙、颌面变化的规律性。方法 :将骨性安氏 II类 1分类错牙合患者 ,以 FMA为标准分为二组 :FMA<3 4°为非高角型组 ,FMA≥ 3 4°为高角型组 ,SNB均小于正常值 ,ANB≥ 8°。采用改良 Activator与头帽高位后牵引 ,高角型组还增加垂直牵引进行治疗。结果 :二组 SNA、SNB、ANB、下颌角、1- 〗 - NA、1- - 1- 、N- ME治疗前后均有显著性差别 ,P<0 .0 1或 P<0 .0 5 ,IMPA治疗前后无显著性差别 ,P>0 .0 5。高角型组 FMA、Y轴角有显著性差别 ,P<0 .0 1。结论 :功能矫治器 Activator加头帽高位后牵引均可治疗生长发育期儿童的骨性安氏 II类 1分类错牙合 ,使面形得到明显的改善。
砄bjective: To study the effects of functinal appliances combined with activator headgear in the treatment of Class II division 1 malocclusion with or without high mandibular angle. Methods: 5 cases (10~12 years old) of class Ⅱ division 1 malocclusion with high mandibular angle (FMA≥34°,HMA) and 8 cases (9~12 years old) without the high angle (FMA<34°,NHMA) were treated with functional appliances combined with activator headgear. All the patients were with low SNB and ANB(≥8). The treatment was continued for 3~14 months and the effects were evaluated by roentgenographic cephalometry. Results:①After treatment, in both groups SNA, ANB, 1- NA and overjet increased significantly ( P <0.01), SNB, mandibular angle and 1- 1- decreased ( P <0.05), S Go and N Me decreased significantly ( P <0.01). ②In HMA group,overbite increased ( P <0.01), while in NHMA group Y axis angle dicreased ( P <0.01).③SNA and ANB increased more in HMA group than in NHMA. Conclusion:Functional appliances combined with activator headgear are effective in the treatment of Class II division I malocclusion of juveniles, but not effective enough in the control of the vertical dimensions in HMA group
