摘要
目的 探讨肺静脉血流频谱对高血压患者左心室舒张功能的评价价值。方法 选择本院门诊或住院的患者 5 5例为研究对象 ,将其分为两组 ,高血压无左房扩大组 3 0例 ,高血压左房扩大组 2 5例 ,同时选择 3 0例无心脏疾患者为对照组 ,用多普勒超声心动图检测各组肺静脉血流频谱 (PVFP)和二尖瓣血流频谱 (MVFP)参数并做比较。结果 与对照组比较 ,高血压无左房扩大组PVFP中S峰速度加快 ,D峰速度减慢 ,S/D >1,AR峰明显加快 ,两组之间有明显差异 (P <0 0 0 1) ;MVFP中E峰降低 ,A峰增加 ,E峰减速时间 (EDT)延长 ,E/A <1(P <0 0 0 1)。与对照组比较 ,高血压左房扩大组PVFP中的S峰降低 ,D峰降低 ,S/D <1,但AR峰明显加速 ,两组之间也有显著的差异 (P <0 0 0 1) ;MVFP中E峰、A峰、E/A比值和EDT两组近似 (P >0 0 5 )。结论 肺静脉血流频谱结合二尖瓣血流频谱能较全面地评价高血压患者的左心室舒张功能。
Objective To discuss assessment of left ventricular diastolic function (LVDF) with primary hypertension by pulmonary venous flow pattern (PVFP).Methods 30 subjects without cardiac disease and 30 patients with hypertension of normal atrium and 25 patients with hypertension of atrial enlargement were studied,all of which originate from clinic and ward in our hospital.The variables of pulmonary venous flow pattern and mitral valve flow pattern (MVFP) between subjects and patients were detected compared.Results Parameters of PVFP and MVFP in hypertension groups with normal atrium were changed significantly,including that S and AR velocity were increased,D velocity was decreased and S/D<1 in PVFP(P<0 001),also including that E velocity was decreased,A velocity and EDT were increased,E/A<1 in MVFP(P<0 001).Parameters of PVFP in hypertension groups with atrial enlargement was changed significantly,but was not changed significantly in MVFP,including that S and D velocity were decreased,S/D<1,AR velocity was strikingly increased in PVFP(P<0 001),also including that E,A velocity and EDT were not changed,E/A>1 in MVFP(P>0 05).Conclusion Left ventricular diastolic function with primary hypertension could be correctly estimated by PVFP and MVFP.
