摘要
目的 研究钾通道开放剂尼可地尔 (nicorandil)在心脏保存中的作用。方法 40个兔心随机分成 4组 ,分别用含尼可地尔的KH液 (超极化心脏保存液 )、UW液、STH液和KH液保存。采用Langendorff灌注模型和工作心模型 ,保存前、后测左室发展压、心室内压上升最大速率、心室内压下降最大速率、冠状动脉流量、肌酸激酶及乳酸脱氢酶 ,记录各组心跳停止和心电活动停止所需时间、心肌含水量、保存后心肌ATP含量。结果 尼可地尔组的各项指标 (除心肌含水量外 )均优于STH组和KH组 ,达到了UW液的效果。
Objective To study the effects of nicorandil in the preservation of rabbit heart.Methods Forty rabbit hearts were randomly divided into 4 groups (n=10 each) and preserved with Krebs-Henseleit (KH) solution containing nicorandil (nicorandil hyperpolarized preservation solution), University of Wisconsin (UW solution), St. Thomas Hospital (STH solution) and KH solution respectively. The perfused rabbit Langendorff model and working heart model were used. Hearts were subjected to 6?h hypothermic (4?℃) storage. Over a wide range of volumes, left ventricular systolic function, diastolic compliance, coronary flow, enzyme release (creatine kinase and lactate dehydrogenase) were measured prior and following storage. Time to mechanical and electrical arrest, post-ischemic percent tissue water and adenosine triphophate concentent after storage were also measured.Results Nicorandil-containing KH group surpassed STH group and KH group, but equivalent to UT group in terms of post-ischimic systolic, diastolic, coronary flow properties, enzyme release and adenosine triphosphate content except post-ischemic percent tissue water.Conclusions Nicorandil hyperpolarized preservation solution possesses efficacy in long-term donor heart preservation. It was equivalent to UW solution. Hyperpolarized arrest with potassium channel openers may be a novel strategy to improve donor heart preservation.
