血栓性血小板减少性紫癜8例临床分析

Clinical analysis of 8 cases of thrombotic thrombocytopenic purpura

目的 :探讨血栓性血小板减少性紫癜 (TTP)的临床特点和疗效。方法 :1996年以前 ,5 / 8例应用泼尼松、肝素、双嘧达莫等治疗 ,仅 1例缓解 ,4例死亡 ;1997年后 ,3/ 8例采用血浆置换 (PE)加泼尼松、肝素、双嘧达莫等治疗 ,3例缓解。结果 :随访 1～ 3年 ,2例健康 ,1例复发 ,死于肾功能衰竭。结论 :TTP为多系统损伤的严重疾病 ,PE是

Objective:To investigate clinical features and therapeutic efficiency in patients with thrombotic thromcytopenic purpura(TTP).Method:TTP patients diagnosed in our hospital during January 1979 and December 1998 were retrospectively analyzed including clinical demonstration,laboratory findings and treatment. Result:Eight patients are 4 males and 4 females with a median age 34 years. All these 8 patients with TTP were characterized by severe thrombocytopenia, microangiopathic hemolytic anemia, neurologic abnormalities, fever and renal involvement proteinuria,hematuria and azotemia. Treatment:before 1996, 5/8 patients received prednisone,heparin, aspirin and dipyridamole,only one case had obvious effect,4 cases died.After 1997,3/8 patients received plasma exchange (PE),prednisone,heparin and dipyridamole. All 3 patients were CR. Three patients were followed up 1~3 years,2 patients were good,no patient relapsed and died from renal function failure. Conclusion:TTP is a severe disease associated with injuring of multiple organism. PE is the best among the therapeutic methods,to treatment of TTP.
