游离腹直肌瓣加中厚植皮修复四肢骨及肌腱外露

Soft tissue repair of the bone/tendon exposure with rectus abdominis muscle flap plus split thickness skin graft in limbs

目的 观察应用游离腹直肌瓣加中厚游离植皮修复四肢骨、肌腱外露的疗效。 方法 应用游离腹直肌瓣加中厚游离植皮修复前臂桡骨、肌腱外露 1例 ,股骨外露 2例 ,足部骨骼外露 5例。 结果 腹直肌瓣均全部成活 ,2例植皮坏死 ,经再次植皮愈合 ,外形及功能尚满意。 结论 游离腹直肌瓣血管蒂长、口径粗、解剖恒定、操作简便、损伤小、无形成腹壁疝的缺点 ,游离腹直肌瓣加中厚游离植皮具有抗感染力强、顺应性好。

Objective To observe the effects of soft tissue repair of the bone/tendon exposure with rectus abdominis muscle flap plus split thickness skin graft in limbs. Methods Eight cases,including 1 case of forearm bone and tendon exposure,5 cases of foot bone exposure and 2 cases of tibia exposure were repaired by rectus abdominis muscle flap plus split thickness skin graft in the past two years Results All the 8 rectus abdominis flaps survived completely without major complication Two cases needed second split thickness skin graft because skin graft were necroses Conclusion The rectus abdominis muscle flap has abundant blood supply and excellent anti infective ability Because the operation is relatively simple and safe,so it is an ideal option when a local or pedicled flap is too small or too far away to reconstruct the defect
