异常勃起的局部注射治疗及其治愈后的勃起功能探讨(附31例报告)

Intracavernous injection of noradrenaline for the treatment of priapism and study of the patients erectile function:Reports of 31 Cases.

目的 总结去甲肾上腺素阴茎海绵体注射治疗异常勃起 ,探讨长时间异常勃起对勃起功能的影响及其他并发症。方法 1988年 8月至 1999年 8月用去甲肾上腺素 0 .5 mg海绵体注射治疗 3 1例 3 6次异常勃起。异常勃起发生时间 7～ 12 0小时 ,平均 2 3 .7小时 ,并对患者作了随访。结果 全部治愈 ,无明显不良后果。4例患者失访 ,2 7例作了长期随访 ,随访时间 8月～ 11年 ,平均 5 .4年。患者短期内勃起功能与发病前无显著差别。但 6个月后 ,异常勃起 >3 6小时的11例患者中有 9例发生勃起功能障碍 ,而 <3 6小时的 16例患者中有 5例有勃起功能障碍 ( P<0 .0 5 )。且长时间异常勃起者中有 4例发生海绵体纤维化。结论 海绵体内注射去甲肾上腺素是治疗异常勃起有效而安全的方法。超过 3 6小时的异常勃起可能导致勃起功能障碍 。

Objective To study the treatment of priapism by intracavernous injection of noradrenaline and complication of priapism after episode of priapism such as erectile dysfunction.Methods 31 patients with preapism(total 36 times of the episodes of priapism)were treated by intracavernous injection of noradrenaline(0.5mg)during Aug.1988 and Aug.1999.The episodes of priapism ranged from 7 to 120 hours(h),mean 23.7h.Results All the patients had been cured without any complications.The 27 patients were followed up for a mean of 5.3 years(ranged from 8 months to 11 years)All the patients had no erectile dysfunction(ED) within 6 months after episodes of his priapism.However,longer than 6 months,9 had ED(82%),and 5 developed penile fibrosis.in the 11 patients who had episode of priapism lasting more than 36h 5 ED(31.25% P <0.05) occured in the 16 patients whose priapism episode less than 36h.Conclusion Intracavernous injection of noradrenaline to treat the patients with priapism is a safe and effective method in despite of the episode length of prianism.Priapism longer than 36h may result in ED and penile fibrosis.
