摘要 目的:应用脉冲振荡法(IOS)探讨大气道内肿物的呼吸阻抗特征和临床意义。方法:测定30例健康人,27例气管内肿物病人的最大呼气流速容积曲线(MEFV曲线)和5~35 HzIOS指标。结果:与健康组相比,气管内肿物组通气指标异常,为阻塞性或混合性通气功能障碍。气管内肿物组的阻力值增高(P<0.01),电抗值减低(P<0.01)。频谱图中阻力线整体抬高,R_(20)明显增高,阻力线在R_5后缓慢降低呈现平台曲线。电抗线右移(共振频率右移),X_5负值增加。结论:大气道内肿物的呼吸阻抗与其他呼吸系统疾病相比,具有鲜明的特点,可为临床诊断提供重要帮助。 Objective: To study the characteristics and clinical significance of respiratory impedance in patients with intratracheal tumor with impulse oscillometry (IOS). Methods: The maximum expiratory flow-volume (MEFV) curve and IOS indices in 30 normal persons and 27 patients with intratracheal tumor were measured. Above data were analyzed with T-test of SPSS statistics software. Results: Compared with normal group, the MEFV curve in intratracheal tumor group indicated obstructive or mixed dysfunction. The resistance values increased and the reactance values decreased significantly in intratracheal tumor group (P<0.01). The resistance curve ascended slowly, whose peak occurred at 5 Hz and then descended to be a concavity. The reactance curve showed right-shift evidently (right-shift of reactance frequency) and the negative value of reactance at 5 Hz increased. Conclusion: The characteristics of respiratory impedance of intratracheal tumor is significantly different from that of other respiratory diseases, which is of great importance to the diagnosis of intratracheal tumor.
