爱普列特对慢性前列腺炎患者尿动力学的影响

The effect of epristeride on urodynamics in patient with chronic prostatitis

目的 探讨 5α -还原酶抑制剂 -爱普列特对慢性前列腺炎 (CP)患者尿道阻力的作用。方法 选择 4 8例患者 ,其中慢性细菌性前列腺炎 (CBP) 10例 ,慢性非细菌性前列腺炎 (CNBP) 38例。给予诺氟沙星 4 0 0mg ,每日两次治疗的同时 ,加用爱普列特 5mg ,每日 2次 ,疗程 4周。用药前后均行尿动力学检查。结果 38例最大尿流率 (Qmax)降低 ,其中CBP组Qmax为 (9.0± 3.0 )ml·s- 1,CNBP组为 (12± 4 .2 )ml·s- 1,治疗前Qmax越低 ,治疗后Qmax增加越高 (P <0 .0 5 )。不稳定膀胱 8例 ,逼尿肌反射亢进 12例 ,膀胱出口梗阻18例。逼尿肌 -外括约肌协同失调 2 5例。结论 尿动力学检查发现CP患者有不同程度的下尿路功能障碍。爱普列特加用诺氟沙星治疗 4周后 ,能降低尿道阻力 ,改善症状。本组研究中未出现不良反应。未发现CBP和CNBP有显著差异。

Objective To evaluate the effect of the 5α-reductase: epristerde on urinary tract resistance of patient with chronic prostatitis.Methods 48 patients were enrolled, in wich 10 patients had chronic bacterial prostatitis and 38 patients had chronic nonbacterial prostatitis . Norfloxacin was administered 400mg twice a day and epristeride was administered 5mg twice a day mean while. The course of treatment was 4 weeks. Urodynamics study was performed before and after treatment.Results The maximum uroflow rate (Qmax) was decreased in 38 patients,in which CBP group was (9.0±3.0) ml·s -1 ,and CNBP group was (12±4.2) ml·s -1 The lower Qmax before treatment,the more in crease after treatment( P <0.05).8 patients had unstable bladder,12 patients had detrusor hyperreflex,18 patients had bladder outflow obstruction,and 25 patients had detrusor sphincter coordinated disturbance.Conclusion The different level dysfunction of the low urethral tract of the patient with CP was discovered in the urodynamics study. Treatment was performed with epristerde associated with norfloxacin during 4 weeks.It could reduce the urinary tract resistance, and improve urinary symptoms.No side effect were found during the test period,but no sisnificant difference was found between CBP group and CNBP group either.
