A PRELIMINARY STUDY ON THE CLINICAL SIGNIFICANCE OF THE VENTILATORY EQUIVALENT FOR CARBON DIOXIDE

摘要 对37例自主呼吸和26例机械通气慢性阻塞性肺病患者作了二氧化碳通气当量(VECO_2)研究。 结果表明,27例动脉血二氧化碳分压(PaCO_2)正常的自主呼吸患者和26例机械通气患者的VECO_2与死腔/潮气量(VD/VT)相关性非常显著(r=0.8355和0.9563, P<0.01),10例高PaCO_2自主呼吸患者的VECO_2与VD/VT无明显相关(r=0.3543,P>0.05)。说明VECO_2不仅反映VD/VT的改变、还受呼吸中枢和呼吸器官机械障碍的影响。 We studied the clinical significance of ventilatory equivalent for carbon dioxide (VEGO3) in 37 spontaneously respirating and 26 mechanically ventilated patients with chronic obstructive pulmonary disease.There was a very significant positive correlation between VECO2 and the dead space/tidal volume (VD/VT) ratios in 27 spontaneously respirating patients with normal arterial pressure of carbon dioxide (Pa002) and 26 patients undergoing mechanical ventilation (r=0.84 and 0.96 respectively,P<0.01).There was no significant correlation between VECO2 and VD/VT ratios in 10 patients respirating spontaneously but with,higher PaCO2(r=0.35,P>0.05).In conclusion,VECO2 not only reflected VD/VT but was also infienced by respiratory centre further and disturbances of respiratory mechanics.
