“双孔”技术治疗二尖瓣前叶脱垂(附20例报告)

Double-orifice" technique for repair of the anterior leaflet prolapse in mitral valve disease

目的 报告“双孔”二尖瓣成形技术治疗二尖瓣前叶脱垂的临床经验和手术结果。方法 自 1999年 3月至 2 0 0 0年 12月 ,2 0例病人接受此项手术治疗 ,其中男 14例 ,女 6例 ,年龄 10～ 6 9岁 ,平均(4 7 1± 18 5 )岁。均诊断为二尖瓣关闭不全 ,其中前叶脱垂 12例 ,前后叶脱垂 8例 ;心功能I级 2例 ,II级 5例 ,III级 13例。结果 无手术及术后死亡 ,无术后并发症及再次手术 ,所有病人均症状消失 ,痊愈出院。随访 0 5～ 2 1个月 ,效果满意 ,心功能均为I～II级。结论 “双孔”二尖瓣成形技术是一种简单、有效的手术方法 ,术后效果良好。

Objective: To report the clinical experience and results of 'double-orifice' technique for repair of the anterior leaflet prolapse in mitral valve disease. Methods: From March, 1999 to December, 2000, 20 patients underwent 'Double orifice' operation for mitral valve disease. There were 14 males and 6 females with a mean age of (47 1±18 5) years (from 10 years to 69 years). Echocardiography demonstrated anterior leaflet prolapse in 12 cases and combined with posterior leaflet prolapse in 8 cases. The cardiac function are NYHA class I in 2 cases, class II 5 and class III in 13. Results: No operative and postoperative death. All patients were clinically asymptomatic after surgery, and were follow up from 0.5 to 21 months. No significant mitral valve incompetence, except slight insufficiency in 2 cases, was found and the cardiac function was significantly improved. Conclusion: The 'Double orifice' technique is a simple and effective procedure. The early outcome are satisfactory.
