0.5%罗哌卡因用于高位硬膜外麻醉临床观察

The clinical effect of ropivacaine 0.5% injected high epidural anesthesia in breast surgery.

目的 0 5 %罗哌卡因用于高位硬膜外麻醉行乳房手术时观察临床效果 ,耐受性和不良反应 ,并与 0 4%布比卡因 ,1 5 %利多卡因比较。方法 选ASAⅠ～Ⅱ级择期乳房手术病人 65例行T3～ 4间隙穿刺后分别接受 0 5 %罗哌卡因 (Ⅰ组 ) ,0 4%布比卡因 (Ⅱ组 ) ,1 5 %利多卡因 (Ⅲ组 ) ,观察显效时间 ,感觉、运动神经阻滞程度 ,术中对循环、呼吸的影响 ,术后不良反应。结果 显效时间Ⅲ组快于Ⅰ、Ⅱ组 ,麻醉平面Ⅰ组稍宽于Ⅱ和Ⅲ组 ,运动阻滞Ⅲ组 >Ⅱ组 >Ⅰ组。结论 0 5 %罗哌卡因适用于不要求肌松的乳房手术。

Objective To observe the clinical effect tolerance and the untoward effects of 0.5% ropivacaine used for high epidural anesthesia in breast surgery,and compare it with 0.4% bupivacaine and 1.5% lidocaine.Methods Sixty five patients in ASA Ⅰ Ⅱ in breast surgery received the puncture in the interspace of T 3 4 ,and then received 0.5% ropivacaine (group I),0.4% bupivacaine (group Ⅱ) and 1.5% lidocaine (group Ⅲ)respectively.The effect rate,the blockade degree of the sensory nerve and the motor nerve,the influence on the circulation and respiration,and the postoperative untoward effects were observed.Results The effect rate of group Ⅲ was faster than that of groups Ⅰ and Ⅱ.The anesthetic plane of group I was wider than that of groups Ⅱ and Ⅲ.The motor blockade sequence of the groups was Ⅲ>Ⅱ>Ⅰ.Conclusion 0.5% ropivacaine is suited for the breast surgery in which muscle relaxation is not demanded.
