摘要
目的 探讨人脑胶质瘤中血管内皮细胞生长因子 (VEGF)的表达与微血管数量 (MVQ)和肿瘤良恶性程度的关系 .方法 用免疫组织化学染色方法检测手术切除石蜡包埋的 47例人脑胶质瘤 (Ⅰ~Ⅱ级 2 2例 ,Ⅲ级 15例 ,Ⅳ级 10例 )组织中的VEGF蛋白表达情况和MVQ数量 ,借助显微镜观察其阳性细胞数和阳性血管数 ;另外 ,分别取脑外伤后开颅内减压术中切除的正常脑组织 10例作为对照组 .结果 ①正常脑组织未检测到VEGF蛋白的表达 ,而MVQ数量少 ,表达强度弱 ;②各级别胶质瘤组织中均有VEGF和MVQ表达 ,主要表达于肿瘤细胞的胞浆和血管内皮细胞胞膜中 ;③ 3组胶质细胞瘤中VEGF的表达阳性率分别为 2 2 .2 %、86.7%、90 %、Ⅰ~Ⅱ级组与其它两组间差异有显著性意义 (p <0 .0 5 ;p <0 .0 1) ,VEGF表达阳性组的MVQ平均值 ( 46.8± 12 .4)明显高于阴性组MVQ值 ( 2 2 .4± 11.5 )(p <0 .0 1) ;④VEGF的表达与MVQ相关 (r =0 .75 ,p <0 .0 0 1) .结论 ①VEGF在脑胶质瘤血管生成中起重要作用 ,能促进胶质瘤血管的形成 ;②VEGF和MVQ与胶质瘤良恶性程度明显相关 ,可作为脑胶质瘤病理诊断的补充指标 ;③VEGF可作为治疗脑胶质瘤的靶向 ,为胶质瘤的基因治疗提供指导方向 .
Objective To investigate the relationship between the expression of vascular endothelial growth factor (VEGF),the microvessel quantity (MVQ) and the grade in human brain gliomas. Methods Immunohistochemical techniques were used to detect the expression of VEGF and MVQ in 47 cases of human gliomas (22 cases of gradeⅠ~Ⅱ,15 cases of gradeⅢ and 10 cases of gradeⅣ) . The positive cells of VEGF and the positive microvessels are examined under binocular light microscope.In addition,10 cases of ablated normal brain tissue specimens from post-brain trauma were used as the control. Results The findings were as follows:① There was no VEGF protein expression in the normal brain tissue ,but there was little MVQ expression in them;② There were expression of VEGF and MVQ mainly on tumor cell plasms and on vascular endothelial cell membranes in all brain gliomas;③ The positive staining rates of VEGF in three group of gliomas were 22.7%,86.7%,90.0% respectively ,there was no significant difference of VEGF between grade Ⅲ and grade Ⅳ ,significant difference was found among the other grades ( p<0.05;p<0.01). MVQ was significantly higher in VEGF-positive gliomas than in VEGF-negative(p<0.01);(4)The expression of VEGF was positively correlated to MVQ in gliomas ( r=0.75 ,p<0.01). Conclusions ①The expression of VEGF protein in brain gliomas is positively correlated to MVQ ,which shows VEGF may play an important role during angiogenesis and promotes angiogenesis of brain gliomas;②The expression of VEGF and MVQ has close relationship with brain gliomas ,and the measurement of the VEGF expression and MVQ may provide one of the predictors for judgement about pathological grade/stage in brain tumors;③VEGF can be used as a treatment target of brain gliomas ,which may provide guidance for genetherapy of brain gliomas.
