胃肝样腺癌的临床病理及免疫组织化学研究

Clinical Pathology and Immunohistochemical Studies of the HAS

目的 探讨胃肝样腺癌 (HAS)病理形态和临床生物学行为特征。方法 对 12例手术切除的HAS进行临床病理学观察及免疫组织化学检查。结果 HAS多见于男性中老年人 ,病灶常位于胃窦部 ,由肝样分化区及腺癌区组成 ,肝样分化区细胞形态及结构与肝细胞癌形态相拟 ,癌肿广泛侵袭脉管 ,预后差。免疫组织化学多数病例AFP、α1-AAT、α1-ACT阳性。结论 HAS是原发性胃癌中较少见的一种类型 ,其病理组织学及临床特征、生物学行为类似肝细胞癌。

Objective To evaluate the pathomorphism and the clinic biological behavior of the hapatoid adenocarcinoma of stomach,(HAS).Methods Clinic pathological observation and immunohistochemical studies were performed on 12 HAS patients.Results Made of hepto-differentiation and adenocarcinoma zone,HAS was often lied in gastric antrum and caught by middle-aged and old patients.The morphosis of the hepto-differentiation zone was similar to that of the carcinoma of liver.The HAS widely infiltrated the artery and vein. lmmunohistochemical studies indicated AFP,α 1-ATT and α 1-ACT were positive in the most cases.Conclusion HAS was rarely seen in primary gastric carcinoma,of the pathohistology,clinical feature and biological behavior of which is similar to that of the carcinoma of liver.
